Milan
 
We are thinking of spending 2 nights in Milan in October as part of a trip around italy.

What would be good to see or do whilst we were there?

I loved walking on the roof of the cathedral.
I also thought the Northern Italians were some of the best looking people I have seen.

See Leonardo da Vinci's The Last Supper. Advance reservations very highly recommended. If you enjoy viewing early Christian churches, visit Sant'Ambrogio a few blocks away.

Visit the museum of the La Scala Opera House or attend a performance there. Walk through the nearby Galleria Vittorio Emanuele to or from Piazza Duomo.

Visit the art collections in the Castello Sforzesco or the Brera Gallery.

Shop.

Not only are "advance reservations very highly recommended," they're necessary (unless you can get into a group tour that's seeing it). Part of the beauty of the Last Supper is that a small group is let in every 15 minutes. You'll never be part of a crowd, and you'll have a significant amount of face time with one of the classic pieces of western art.

Yes, I missed the Last Supper too when I was in Milan in May. That stupid book messed it up for us.
