If you require a rental car that has automatic transmission, they're rare in Europe, including the UK. Your best bet is an airport office or one in a big city. In smaller places, they may promise you a car with automatic transmission, but when you arrive, none is available.
For driving in Italy, you should get an International Driver's Permit, to go along with your license. In North America, an IDP is easily and cheaply available at your local AAA/CAA office. Costs about $25 including photo.

Here's another idea - fly into LHR [I assume] spend 3-4 nights in London, then hire car, and tour Kent and/or Sussex [loads to see and do, beautiful countryside, castles, gardens, stately homes, coastal scenery] then return your car to LGW and get Easyjet or Ryanair to your Italian city of choice.
