Italy slurge
I was hoping to plan "one" splurge (fabulous hotel) in Italy. I realize everyone has different tastes, which city and hotel would you splurge for?

When you choose your splurge remember that your splurge money will go a lot further in some places in Italy than in others. For example, a modest hotel in Venice may seem like a splurge. For your itinerary I think Hotel Lungarno in Florence would be my splurge. The other locales are splurge enough.

I'm going for my honeymoon 16 nights:
1) Venice
-Maybe Lake Garda
2) Florence
3) Tuscany
4) Positano
-Maybe a few nights in Ravello
5) Rome
How do you suggest I spend my 16 nights? I'm open to your recommendations. We have a reasonable budget $500+ per night for hotels.

For your "one splurge" you mean to spend even MORE than $500 per night?
If that's your average nightly hotel budget, you can easily splurge every place you stay.

This price range is out of my league, but I'd suggest looking at the Luxury Hotels forum at Flyertalk.com .
Also, if you are planning to spend about 400 Euros per night, consider whether you want an "ordinary" room at a very "top" hotel, or perhaps a suite at a slightly "lesser" hotel.
I seem to recall that there are special deals with AMEX and getting upgrades with certain luxury hotels, but you can no doubt find that out at Flyertalk.com .

Have a look at the websites for Travel & Leisure and Conde Nast Traveler - they both have annual best hotel in the world lists (compiled from reader opinions/experiences) . I've stayed at seveal of those listed for business (and even one or two for vacation) and they have all been fabulous.
