If a doctor in Italy wants cash, it's for tax avoidance, and you won't get any legal receipt. I refuse to deal with doctors who want cash. Aside from the morality of it, I worry that doctors who are dodgy with their taxes may not be reliable with their medicine.

If it was your idea - re-think it. A credit card makes more sense and will likely cost less in the long run.

You should be nervous carrying that much cash. It is not a smart idea.

And I don't think you understand the fees and bad rates that maybe associated with exchanging cash for cash. As others have pointed out your credit card even at 3% may be the better deal.
