Help with trip in Nov (Milan, Cinque Terre, Pisa, Florence, Rome)

Hi, there are 2 of us in our early 30s and we are interested in nature, scenery, food, culture. We have slightly less than 2 weeks, and will be flying into Milan, and out of Rome in the first two weeks of November. We need some help with the sequencing of places; we will not be driving. Grateful also for any tips or any recommendations for picturesque small towns or day trips that could fit into our itinerary (especially from/to Florence or Rome).

Day 1: we land at 6am. Sightseeing in Milan (night in Milan)
Day 2: day trip to Lake Como (night in Milan)
QUESTION: is a DIY day trip to Lake Como manageable, or should we join a day tour?
Day 3: we intend to do the Bernina Express Rail day tour (night in Milan)

Day 4: Cinque Terre (train from Milan to Monterosso?) - night in CT
QUESTION: does it make more sense to visit CT from Milan, rather than do Milan --> Florence and then join a day tour from Florence?

Day 5: Pisa (night in Pisa?)
QUESTION: should we head from CT to Florence, and do Pisa as a day trip from Florence instead? Saves moving hotels so many times, and we only want to see the Cathedral and Tower of Pisa.

Days 6, 7, 8 (3 nights in Florence)

Days 9, 10, 11 (3 nights in Rome)

Day 12 - morning flight out of Rome

my only thoughts are 5T in November! May well need hiking boots
