Taxi.
I use G7 (http://www.taxisg7.fr) when I have a specific need. You can dial 3607 on your telephone or reserve the taxi online. I have never had them arrive late.

I use Taxi G7, also, and find them reliable. In fact, they have a special number for English speakers, if you want that. It is 01-41-27-66-99
It's listed on their website in the commander un taxi section.

At such an early hour you may well choose to take a taxi all the way. However, if saving money is important to you, you could take a taxi to either Gare de l'Est or Gare du Nord and catch a Noctilien (night) bus to CDG. N120 leaves Gare de l'Est at 04:30 and Gare du Nord at 04:33. It arrives at Terminal 2 at 05:10, Terminal 1 at 05:15, and Terminal 3 at 05:20. N121 leaves Gare de l'Est at 04:25 and Gare du Nord at 04:28. It arrives at T2 at 05:05, T1 at 05:10, and T3 at 05:15.

Another option would be to take a taxi to Gare du Nord in time to catch the first RER B train to CDG. It departs at 04:56 and arrives at CDG 1 at 05:28 and CDG 2 TGV at 05:30. Kerouac will know for sure, but I believe this particular train at that time of day departs from the surface rather than from underground.

The first RER B train from Gare du Nord departs at 4h56, arriving CDG TGV 2 at 5h30. That should be plenty early, because I doubt you'll need a full two hours to get on the first flight of the day.
A night bus can get you to GdN from anywhere in the city. http://www.ratp.info/informer/noctilien.php
