Len,
As you were instructed by lebara, I ended up putting city, state, country and zip. I received my sim a month later. Since no one documented exactly how the address is mapped between the online form and the envelope, here how it came out on the envelope. It ended up in 3 lines.

line 1: [street number/name]
line 2: [postal code] [city]
line 3: FRANCE

The 3rd line was generated by lebara. I did not type it.

As you can see, the [postal code] field is at an unusual location for a US address. The city field has to carry the state, country, and the US zip code as you were told by the lebara agent. Somewhere in the process, someone crossed out "FRANCE" using a ballpoint pen.

FRANCE was not crossed out on my envelope. On one of my SIMs I put the zip code for Nice France - and it got to me at the same time the one with my correct zip arrived.

A few things I learned with the Lebara.
-I could contact to lebara.fr from my phone via Paris hotel Wi-Fi bit not from my tablet. It just hang with blank screen.
- The offer show on the promotion page such as 6hr talk, 1gb for 14.99 cannot be directly purchased. I had to to up first, but they only gave me fixed options. I chose 20eu with 1gb bonus. Only then I can send "6HR" promo SMS to 22241, listed on promo page. I ended up with 2gb for 1 mo including promo.
- The data did not connect automatically. Lebara is a SIM operator and not the net carrier. The actual call is handled by Bouygues. The default apn Bouygues does not work. Web searched on Wi-Fi and found it has to be manually configured!

I ordered free Lebara SIMS last trip (2012) on the website and they came with no problem. This year I ordered a free SIM, and it never arrived. I ended up ordering one from Amazon at the last minute for about $10. It came and I loaded €10 of time on the Lebara website. It worked great - I used it quite a lot and still had plenty of time left after three weeks. Don't know why the free SIM never came, but it was worth $10 to get it from Amazon.

Lebara may be keeping a record of to whom they send SIMs. I don´t know of many people who have received a free SIM more than once.

We ordered 3 SIM cards this year under two different names. We received two of them. Didn't receive the one where we ordered two under the same name. We ordered a SIM each of the last two years and received them both - so that's three with the same name in 3 years - but only 1 of the two ordered this year under the same name.
