Let me clear up some misconceptions, I have read on this thread. If you have 3rd party insurance except medicare and medicaid (ie -pvt. insurance), you are responsible for your health care bill while abroad, and they will reimburse you upon return. This is good as long as your stay was not over 6 months out of the states. So if you are a US citizen who has retired in France, your insurance will not cover you as long as you are not in the states 6 months out of the year.

I have purchased trip insurance on fairly expensive tours because I have a preexisting condition and if it ever becomes a problem, I would like to get my money back and be flown to a hospital if necessary. I would not purchase the insurance from the tour operator. If you are using a travel agent I would purchase it from them. If you are not using a travel agent then check very carefully because I could not find any insurance that would cover me if the tour operator went belly up. You have to ask the questions because it is not easy to find in their information.

The other reason to purchase the insurance is for health reasons -- the reason I have felt better purchasing it when I have laid out a lot of money -- preexisting conditions are covered if you purchase the insurance at the time you purchase the tour -- or within usually 2 weeks. You can up the insurance coverage later if you need to so look at what you are prepared to lose before you decide how much to cover.
