We rented a car in Scotland last year and I declined the extra insurance as my credit card covers for collision and theft. I was given a hard sell at the counter to buy the extra insurance and was told that they would put a large hold on the card (the price of the car)! I still did not accept the extra insurance. I was a bit worried that we would max out the card (but we always carry another one) but had no issues on our two week trip. Not sure if the hold was really placed or it is a sales tactic. I almost gave in and bought the extra. My husband is not such an easy sell.

I've just been doing a Google image search of various state drivers licenses. Looks like California is not using the international format either.

It is such a simple format to apply. I don't know why some places are resisting.
