In Canada, I used NUVO - about 3cents/minute to most US, and Canada numbers (both landline, and mobile). For $10, I recieved 276minutes. The card lasted 3 months traveling through Canada, and the U.S.
I'd like a similar card for Europe. What do you recommend for the lowest rates/minute, and with a toll-free number.

I always buy a Telefonica Phone card, the kind with the I/C Chip on the card itself: that way you don't have to call an '800' number for access. Most pay phones in Spain are Telefonica, so there's no problem finding one.
A direct dial call from Spain to the U.S. using the card was less than 10-cents (U.S.) a minute.
Buy them in a Tabac or at some news stands

10eurocents/or 10cents/minute is too much.
Perhaps calling cards with a 1-800-number will be cheaper?
How will I know how much the calls will cost (to within Spain, and Europe). I have no intention of calling North America or anywhere else.
Will the rate/minute be listed on the card?

Agreed; but I wrote: "less than 10 Cents (US) a minute!"

My experience: with direct dial calls, 3 cents /min to call Madrid or Huelva from Santander.
"Will the rate/minute be listed on the card?"
No! But the view screen on the Telefonica phone will give you an updated balance each time you insert the card.
