Just to point out that :

A little expresso with milk is "un petit creme" une "noisette" is the same but with very few milk

And "une menthe a l'eau" is mint syrup with tap water while "un vittel menthe" is mint syrup with mineral water (Vittel is the brand of the water)

I'd recommend you buy a travelers' phrase book instead of copying down some words to paper as there are just so many possibilities on a menu. I'd suggest Dover's Say It in French--it's only about $3 and has a very good "restaurant" section (food and drink, menu terms)--the entire paperback book is smaller than a post card (and only about 1/2" or less in width. You can easily carry that in your pocket or purse, I think it will be more useful and easier to find things (not to mention it includes all kinds of other phrases). Their food section is arranged alphabetically by the French term (within categories, such as drinks, meat, etc). Alternatively, there are food-only phrase sections in a good restaurant or food guidebook--you could just Xerox the few pages. Patricia Wells' A Food Lovers Guide to Paris has a good one, and I think Cheap Eats in Paris does also.
