If this is your first Mediterranean cruise, I would start on the east side.

How mcuh time do you have? What do you enjoy most on a port stop? Culture? History? Food? Wine? Shopping? Church? Natural scenery? Beaches? The answers would dictate the destination.

We did 2 Carnival Mediterranean cruises, both Western. Round trip Rome and Rome to Barcelona.

The 2 combined (21 days) covered Rome, Naples, Florence, Pisa, Capri, Amalfi, Pompeii, Sicily, Venice, Dubrovnik, Palma, Monaco, Nice, Aix En Provence, Marseilles, Cannes, & Barcelona. Some were duplicates of places we had been before on land visits but it's a great way to see lots of different places and than go back to those you really enjoy to spend more time.

Being on Carnival, the price were quite reasonable.

We need more info: how long, are the places you really want to go, how old are you, one way or RT, when? We've been on a 21 day Barcelona, Malta, Egypt, Greece, Malta and are taking a 18day in October going from Venice to Rome, with stops in Tunesia, Croatia, Monaco, and Italy.

So it just depends.

large ship with a couple of thousand people or smaller with maybe 500 people. Makes a big difference in your experience. Luxury or cattle call? Lots of activities on board ship or are you in it for relaxation and shore excursions? Personally we only go on smaller ships. Did the big ship thing once and found out it was not for us. we don't care if there are bumper cars (see the Quantem Leap cruise)or golf or Las Vengas style entertainment but that is important to some people. You have to serious think about your preferences. Larger ship, less cost but way more people. The mediterranean is a good start Spain to Italy as it hits some of France.
