Well, you can charter a small plane to take you almost anywhere in the Caribbean as long as you are willing to pay the going rate. Keep in mind that when chartering a plane (the only way the flight "will tailor its schedule to your arrival" you will have to pay not only for your flight TO the destination but also for the plane and its pilot to get BACK to the home base). Figure something like $750 to $1,000 for a one way charter flight lasting about 30 minutes.
Of course, there are numerous options using scheduled airlines so if you have budgetary considerations that may be your best bet.
All that said you'll find many islands can be reached in relatively short flights from SJU.
For starters, check out Vieques or Culebra (about a 15 minute flight from SJU). A 30 to 40 minute flight can get you to Virgin Gorda or Anegada. In the same amount of time you can also fly into Beef Island (Tortola) then take a boat or ferry to just about all of the British Vigin Islands. The USI's are about 30 minutes flying time from SJU. Consider flying to St. Thomas then take a ferry to St. John.
If by quick flight you mean under an hour than you can add Anguilla, St. Martin, St. Barths, St. Kitts and Nevis to your destination list.
