I went to China during my second trimester with our first. It was a great trip. Our OB suggested we go -- he took his wife to Italy in her second trimester with twins!!! Talk to your doc -- the one thing ours said was to drink a ton of water on the plane and walk around a lot. I strolled the aisles about every 90 minutes during the 15 hour flight and did lots of stretches and was fine. Climbed the Great Wall, ate all the food, had a ball. The only issue and something I totally did not think of before we left was the pregnant need to use the restroom all the time. China is, shall we say, not known for its elegant and clean public restrooms. THAT part of the trip was NOT FUN.
Anyway, one thing I would say is know your comfort level with health care and what level of competence and technology you need. We are very spoiled in the US with all the latest and greatest technology and techniques and come to take it all for granted and consider an abundance of tech a minimum baseline. We were just in the Caymans with our three toddlers and had to go to the ER twice during our one week trip!!! On one visit (suspected DVT -- turns out just a ruptured cyst) our experience was quite good and we had lots of faith in the MD. On the other (our three year old had a horrible reaction to a bug bite) the pediatrician did not instill any confidence at all, suggested drugs and techniques I never had heard of in the US (I am in the health care field) and had it not been for many, many consultations with our pediatrician back home, we would have been on the first flight out to the states (Miami, Houston, whereever we could fly) to have him seen in a US hospital. So, just know what level of health care you personally need and plan accordingly. If my husband or I had been the ones with the bad reaction, we probably wouldn't have cared but as this was our precious child.....
Have fun.

I would highly recommend Caneel Bay in St. John also. My DH and I went there years ago before we became parents and we still talk about it.
I was so gorgeous and relaxing, it is perfect for couples or honeymooners.
We honeymooned in Hawaii which was wonderful but after going to St. John we wished we had honeymoooned there instead.

Thanks, this is a great thread.
We'd prefer to go to St. John to Puerto Rico, but am I right in thinking that the health care in Puerto Rico would be superior to St. John / St. Thomas should anything happen?
We'd be at a resort an hour from San Juan, which would be a similar distance that St. John would be to the St. Thomas hospital (though a ferry is involved.)

As I stated prior, if the Dr. gave the greenlight, we went. Oftentimes the trips had been planned even before we found out we were pregnant. My wife worked both days that she delivered until 4 pm or so. I can't see how a relaxing vacation can be a detriment.
