Where might I obtain a couple of hand-made linen shirts and what might they cost?

if you are speaking of bkk, two places come to mind and they would be $20-40 probably:

js thai silk, ground floor, river city shopping center, next to sheraton orchid hotel

cotton house, in OP Place which is a small shopping center directly behind the oriental hotel---ground floor on the left with the hotel to your back--there are two corridors...

For cheaper you can get tailor made cotton shirts at the tailors around Asia Hotel, Bangkok for around US$13 onwards.

remember that the fabric content is always a question....merchants will claim it is 100% cotton or linen, but in fact it is not....that is why you want to go to a known tailor... i would be suspect of a $13 shirt??
