Omakase in Tokyo and Kyoto

Hello,
I will be traveling to Japan in just over a week, and would appreciate suggestions for a restaurant in Tokyo or Kyoto with an outstanding Omakase chef's tasting menu. This will be my splurge meal and am willing to spend up to $150 per person, if that helps.

Thanks!

I'm not sure this meets your needs, but my splurge dinner in Japan was at a French kaiseki-style restaurant in Kyoto. I believe it qualifies as omakase, as I simply specified price/number of dishes, and the chef took it from there. It was the Misoguigawa in Pontocho:
