NUANCED food allergy cards in Thai?

Back in the day I traveled annually to Thailand to visit son working there, and visited this forum a lot. I believe it was here that someone prepared and provided a set of food allergy question and warnings translations in Thai that was far more detailed and nuanced than what is available on translation/health/Thai tourism sites these days. Does anyone have a link to that list? or a copy of the PDF? We're talking 8-10 years ago, probably. (I have the links that come up with a google search but I'm looking for the individually prepared list that I think was posted here.) Thanks!

I download food cards from celiactravel.com, but that's only related to gluten allergies. You can take the write up they use (and change the allergy information ), then use google translate to come up with your own. Your son can show it to someone when he gets there and tweak it accordingly.

Just a thought.
