So you are fly on JAL for HND->SFO. Is that an award ticket?
Why are you considering a 5PM flight from ITM to HND when JAL has two later flights that would cut your connection time down from 6 hours?
Kadensho would give you a kind of onsen ryokan experience. It is a ryokan hotel, so a bit different than a traditional ryokan. This kind of lodging has its appeal and Kadensho looks like a nice one.
If you really want a traditional onsen ryokan experience and get some quality time with Fuji-san then I suggest that you skip Nikko and go to Hakone instead and spend the night there.
You can certainly find a nice ryokan on Miyajima (kja will recommend Momijiso and so would I) but an onsen ryokan? I am not so sure. At least, there are more options in Hakone.

mrwunrfl - The later flights weren't from ITM to HND weren't available on our award ticket. I've been calling weekly to see if any seats have opened up on the 8:30 flight.
I think I'm going to leave the itin as is. We'll see how we feel about the day trips to Nikko and Nara when we're there.

If you do get a later flight or plan to fly to HND with your existing reservation on Day 10 then it would make sense to activate your 7-day JR Pass on Day 3 for Nikko. Either way, you definitely will save with a JR Pass.
