How do you get to Ginza, Shinjuku, Asakusa and Ueno from Shibuya?

I'm in the early stages of planning a trip to Tokyo but the map in my Fodors book doesn't give me much of an idea how far away places actually are from each other and how to get there. Is there a subway to get directly back and forth? Is there a website that shows transportation?

I'm thinking about staying at the Cerulean Tower in Shibuya for 5/6 days and then stay at Disney (Miracosta) for 3/4 days. How far away is Disney from Shibuya? What kind of transportation would we need to get there?

This would be our frist trip to Asia. Any suggestions you can give me would be appreciated. :)
