To the best of my knowledge, other than an around-the-world cruise which may include Bali as part of the overall route, there are no regular cruise routes from the US to Bali. However, I believe you can take what are called “repositioning” cruises, which are when the cruise company needs to get their ships from one place you another and the end of a cruising season. I believe some of the Alaska cruise lines have repositioning cruises in September or October at the end of the season in Alaska, when they need to move ships to south. The prices are usually less than they normally charge, but I am not sure they offer all the lectures and on-board entertainment that they would offer on a normal cruise. I would suggest you check with the websites of major cruise lines and see if they have any repositioning cruises. You could also Google “repositioning cruises” and see what comes up. You should also try the cruise board on this website or go to cruisecritic.com.

I've never heard of either. Like Cicerone says, you might find an around the world cruise that includes both, but otherwise you need to fly somewhere closer to Bali to get a cruise with Bali on its itinerary.

You also could look into freighter travel. I know you can get at least as far as Singapore from the US on a single ship (it's 17 or 18 days IIR). Then pick up something from there or Jakarta maybe. Or just fly those last couple of miles.
Freight travel, while still on the ocean, is quite different than a cruise ship of course.
