3 week itinerary help
 
My wife and I are arriving in Osaka on the 25th March and departing Tokyo on the 15th April.
We will be using the rail system for travelling between the destinations.
Advice required about length of stay in each place.First thoughts
osaka ( or stay in Kyoto/ Kobe and travel between),hiroshima,Kanazawa, Takayama,Tokyo.
Other places that have been mentioned Matsuyama , Nara, Nagano.

Should I buy a 3 week rail pass?

IME, typical lengths of stay for first time visiter include 6 nights in Kyoto (to include Nara, more if you want to include Osaka and/or Kobe); 5 nights for Kanazawa and Takayama (split 2 and 3 according to your interests), any where from 3 to 7 nights for Tokyo, depending on your priorities, and 1 or 2 nights for Hiroshima & Miyajima. You can search this board for planning threads -- there are a LOT.

If you haven't already done so, spend LOTS of time with japan-guide.com

It would be best to finalize your itinerary before deciding on a rail pass.
