My wife and I plan this general itinerary for 16 days in a camper. Is this easily doable? Any tips for a first time visit?

Why in a camper? When?

National park accommodations in S Africa are very affordable. You can also stay in affordable guest houses in most towns. A camper seems unnecessary, in my opinion. In 16 days, going to Lesotho will make it a drive by, you won't get to see a lot, or other areas will get a drive by. I have driven JNB, Kruger (stay as many nights as possible, we stay 4-8 nights no less than 3 as its the premier game reserve), Swaziland one night or more at one of their excellent game reserves or in Mbabane for casino and luxury. Then Mkuze (one or two nights) and Hluhluwe-iMfolozi regional game reserves (equivalent to national parks) for three nights. Add a few nights in St Lucia (my favorite fun town) in a guest house or Cape Vidal on the beach. The iSimangaliso Wetland Park is gorgeous and so much to do from Kosi Bay to Cape Vidal.

You can visit the midlands and drakenbergs on your way to Lesotho for a few days. They are beautiful and history filled. The towns and beaches between St Lucia and Durban are pretty. Zululand Rhino Reserve and Tembe Elephant Park are both beautiful affordable game reserves if you need a little full service game viewing to truly get away from it all for a few days.

Not sure why you want to visit three countries or what you want to do. Do less, enjoy more. Choose what's important and enjoy it. Don't just drive through. You can also easily and cheaply fly back to JNB from Durban, saving a full day of driving. Just drop off your rental car - no camper needed - and fly.

I am planning to do Kruger, Swaziland (just one night transit through), Zululand Rhino Reserve (we stay at Rhino River Lodge - affordable, great service, super friendly, great game viewing) and St Lucia (plus a little real estate dream shopping!) soon. It's a great trip. But that absolutely fills 14-20 days. I don't add more unless I have much more time or (no!! Impossible!!) drop something else.
