Suggest 5 day itinerary Dubrovnik--Lakes--Split?

Suggest 5 day itinerary Dubrovnik--Lakes--Split?
We will be spending 5 days in Croatia from early morning of September 30-, arriving by ferry at 7:00 am by car, departing on October 5 early morning from Split. We would also like to see Plivice Lakes National Park. Can you suggest the best itinerary. If possible we would also like to visit the Island of Hvar. Do ferries run all days in October?

Loved Split old town, promenade and surroundings!
would skip Dubrovnik(full of tour ships and limited to the old town)
would skip water falls too. Lake Bled did not excite me either.

Am I correct you arrive at Dubrovnik and depart from Split (both by ferry, with your own car--or are you flying out)?
If so, I would suggest spending the night in Dubrovnik (you'll want at least one day there--it is tiny and yes the cruise crowds overwhelm it, but it is as lovely as you've heard, especially in evening. Seek out side streets. Parking may be an issue, choose accommodations accordingly.)
The next day, take a leisurely drive north. I have not driven this entire stretch (mainland between Dbv and Split), but I suggest researching a place you'd like to stop in between on the mainland. I am not suggesting the ferry, because it is a long boat ride when you could be exploring.
So, that's a second night of your choosing. (I advice looking at viamichelin website to plan your driving routes.)
The next day, continue on your drive to Plitvice. This park is a true wonder. One night there.
The next day, return drive toward Split, Sibenik would make a nice stop. Remaining 2 nights in Split/Trogir region. Take day trip (with car; ferries are daily from Split) to Hvar.

A day in Dubrovnik. is enough.But well worth a visit to the old town. & it won't be very crowded at that time. As you have such a short time .Drive to Plivice Lakes it is really beautiful
Split is OK for ferry to Havar. go to www.visit-croatia.co.uk
for good info on ferries etc, Trogir is very nice about a 45 minute drive from Split.
