Barcelona 5-day Itinerary
Day 1:
Hospital de la Santa Creu
Sagrada Familia
Casa Batllo
Casa Mila
Night stroll on Passeig De Gracia
Day 2:
La Rambla
Mercat de La Boqueria
La Seu Cathedral
Esglesia de Sants Just i Pastor
Esglesia de Santa Maria del Pi
Day 3:
Museu Picasso
Palau de la Musica Catalana
Esglesia de Santa Maria del Mar
Parc de la Ciutadella
Dinner in El Born
Day 4:
Park Guell
Camp Nou
Explore La Gracia neighborhood
Day 5:

What are your thoughts? I might have to cut it down by one day. Would I still be able to fit everything within 4 days?

Day 1 is overcrowded with sights that take at least a couple of hours each and are too much alike. Move one of the houses things to another day and go first thing in the morning to have fewer crowds. I found the school groups to be annoying as the kids were only interested in flirting and texting. Weekends would be better at La Pedrera and Casa Battlo.
Do you mean the Sant Paul Hospital (the UNESCO site)? It's under reconstruction and you can only see a limited part of it. If you're going next year then more will be finished.
Parc Guell - go during the week when there are fewer people.

Thanks for the tips Adrienne, actually day 1 happens to be a Saturday. So Maybe I need to move Museu Picasso to another day since it falls on a Monday.

Considering their proximity to each other, splitting Casa Battlo and Casa Mila into different days makes no sense. But those two plus Sagrada Familia (which is not close to them) is an enormous lot for a day.
Why Camp Nou? Do you follow Barca? Do you follow La Liga? Otherwise, it's a stadium for association football.
Three churches on a Sunday means disturbing the faithful at some point, no? Also means crowds - Spain's still Catholic, so is Catalonia.
La Rambla is overrated, so is the Mercado. Rambla de Catalunya is a nice place for dinner. Or if you're going to drop serious money, Port Vell. Barceloneta is on the other side of the city from Montjuic.

We are futbol fans and really enjoyed visiting Camp Nou. In addition to seeing the stadium we got to see the locker rooms (simple compared to NBA or NFL) and the chapel. Probably the most interesting part was the museum they had with historical items and a fascinating photography exhibit.
Be sure and get tickets a few days in advance for the Palau de la Musica for the English tour. We tried to get them earlier the same day and they were sold out.
We were there for 5 nights and I agree that you may be packing in a bit much, definitely don't think you could squeeze that list down to 4 days. Here is a link to my trip report, you can see what we accomplished each day and there are also pictures:

Agree with Robert that this is too busy, but I like your ambition to make this a thorough Gaudí tour. The devoted Catholic Gaudí holds many of the keys to the development of this modernistic city. And I strongly recommen to buy tickets online in advance to escape the often very long waits in line at La Sagrada Famila, La Pedrera (Casa Milà) and Casa Batllo.
You should also have a rest at Plaça Reial, just off Las Ramblas, where Gaudí, as his first assignment in Barcelona, designed some quite beautiful lampposts.

Wow, and I thought my itinerary was slow-paced compared to the suggested itinerary at Frommers. The latter has Park Guell, Sagrada Familia, The Cathedral, Barri Gotic, La Rambla and Motnjuic in one day.
Is it just day 1 that's packed? I think I'm gonna skip Picasso Museum on this trip. The other churches are part of a walking tour. I won't spend an hour in each.
Is there anything else worth seeing that I might have forgotten?

Be sure to see Sagrada Familia inside..
At he doors of Santa Maria del Mar, you can see how the "bastaixos" transported the stones needed to build the church...
If possible, read "A Cathedral by the Sea", the novel about Santa Maria del Mar, obviously a church,,not a cathedral.

danon - the link you gave was for a completely different hospital. It's for the one that's under reconstruction and is a UNESCO site. I was at that one too. Please take a look at my link and the link the OP gave.

Exactly. You can tour the inside early in the day. The OP wants to visit so he should put it on day 2 with the other things in the area, if the hospital is open then. I didn't think it was near Sagrada Familia at all.
