There are nonstop flights from Atlanta (Delta, seasonal), Charlotte (US Airways), Miami (American), New York–JFK (American, JetBlue), New York–Newark (United), Philadelphia (US Airways), and Boston (JetBlue). There are also some nonstop charter flights (including GWV/Apple Vacations from Boston). You can also connect in San Juan, primarily on American. Many smaller Caribbean-based airlines, including Air Caraïbes, Caribbean Airlines, Dutch Antilles Express, Insel, LIAT, and Winair (Windward Islands Airways), offer service from other islands in the Caribbean.
