You'll get much better responses if you post on the Thailand board. This is a very general and rarely used board.
Fees are generally dependent on your card. Some cards charge a percent, some a flat fee. For an ATM, a debit card that has low fees for using an out of network ATM and/or low/no foreign transaction fees is what tends to work best. Check with your bank/credit union for what fees their cards charge.

As above, fees depend on your own home bank and who, if any, banks they are affiliated with in Thailand.
