Budget? Five star or three? What is important to you for hotels. Location, price????

We are open.

Sorry, but it's very hard to make suggestions - in Cusco, you can spend $25 and you can spend $300, and they'd both be "good, comfortable, clean safe location".

I like the following hotels. Whether they will work for you is impossible to say given your vague requirements:

Lima: 3B Barranco, Second Home Peru
Cusco: El Balcon (note, lots of steps) http://www.balconcusco.com/en/cusco-hostel-el-balcon
Ollantaytambo: El Albergue
Puno: Qelqatani or Don Giorgio

You might also look at this small hotel chain

We stayed at El Albergue, and really liked it. We got a very good last minute price on the hotel's own website.

I would stay in Ollantaytambo rather than Urumbamba. Apu Lodge is definitely worth a look, off the main square. Huge rooms and great views of the ruins

Prices as of last May:
Lima: Miraflores Inn - $48US/night, double room
Puno: Hotel Kuntur Inn - $50US/night, double room
Cusco: Ninos Hotel (Meloc) - $53US/night, double room

I think you can typically stay 2 to a room, so it might be awkward with 5 (2 rooms? 3?)

It is pretty common to find Triple rooms in Peru--3 twin beds.
