Want to do the Km 104 hike up to Machu Picchu and know we have to arrange in advance with a guide. Also think we'd want a guide for the ruins. Any suggestions? Any companies you recommend?

You actually need to book through an agency that has a licence to operate on the trail rather than direct through a guide, so that means booking the whole package, transport, train, entrance ticket, guide etc.

Yes, you must select a liscensed company to operate the Inca trail. There are many recommended companies here or frommers, or trip advisor...Go with a company you feel comfortable with..
