Flat bed seats

I plan to fly to Rio de Janeiro in March, 2014. I have a few questions:
1. Which airlines fly directly to Rio from Los Angeles (ie do not stop first in Sao Paulo)?
Stop in Dallas/Fort Worth is OK.
2. Which airlines have flat bed seats in Business class? Can you definitely reserve a flat bed seat? I need the seat for my back and do not want to go (and pay that much Business Class) if I cannot lie down.
