You should wear clothing in which you feel comfortable, the same clothing you would wear at home for similar activities. That said, I feel Italians dress up a bit more for everyday activities, more like every day is "Casual Friday" at work.

What you pack depends on time of year, what you'll be doing, and where you'll dine.
For example, if you'll be hiking around Lake Como or Capri you would pack appropriate hiking gear.
If your focus is on theatre/opera and fine dining your wardrobe would be a bit more upscale than the tourist who does not go in for these things.
For standard day-time sightseeing slacks/skirts and a tee/polo shirt with good walking shoes is fine.
