There are frequent public buses from Linate airport to Milano Centrale railway station.
From there you can either
board a train to Varenna (every 2 hrs) and cross the Lake up to Bellagio by ferry (about every hr)
or
board a train to Como (about every hr) and go on by Bellagio bound bus (irregular timetable intervals, about every 2 hrs)

There are numerous agencies you can find by Googling that can provide a transfer in a van from any of the Milan airports to just about anywhere on the lake, or at least to a more convenient ferry point for Bellagio such as the one in Menaggio.

I went from Centrale to Como by train, then took the ferry to Bellagio. The long approach as Bellagio slowly came into view was worth it for this first-timer. Really beautiful. Hope your trip is grat!

I think that such an experienced traveller as Bob is fully aware of the various standard connections suggested.
I do not know of any direct Linate/Bellagio service. I don't believe there is the demand for it.

Linate is going to become a major hub for Etihad following their tie-up with Alitalia.
Business and first class passengers have complementary chauffeur service from Malpensa (http://www.etihad.com/en-au/experien...had-chauffeur/) so it may also be introduced for Linate.
The limit is 60km from the airport of Malpensa. Bellagio is out of range for both Malpensa (113km) and Linate (84km). However, if travelling as a couple they might be prepared to extend the range.
At present direct US routes are not known but it is likely that some routes will be announced. Emirates already offers a direct Malpensa - New York route.

OP is looking for a van or bus service. Not a private driver and not a van or bus used by a group.
I know of no regular van or bus service from Linate to Bellagio. or from Malpensa for that matter.
There used to be a Lufthansa bus from Como to Malpensa but nothing to Linate.
