taxis in Italy

I'm leaving for Italy in one week. I won't have a rental car and will need to occasionally hire a taxi.

I don't want to get ripped off, but I have little to no experience with cabs.
What do taxis in Italy look like?
Any special markings or licenses to look for?
Do I need to negotiate a price before I get into the car?
Should I expect to pay a luggage handling fee?
