I am going with my SO in mid June. I've only been to Spain once - 5 years ago. I went to Barcelona (4-5 days) and drove up and down La Costa Brava (3-4 days).
I love Barcelona and I have family there, so I'd like to visit again. From there, I'm not sure what's best. I'd like to check out Madrid, Sevilla and San Sebastián (and perhaps drive around the basque areas and make a pilgrimage to my ancestry in Santander and Motrico).
I am totally flexible - don't mind trains, driving, etc. I'm up for anything but want to understand if my itinerary is reasonable.

I think 4 cities in 10 days is a bit much. I've been to Spain a couple of times and there is a lot to do in the cities you named. Last year I visited Madrid and Barcelona in 8 days and I wish I had more time in just those 2 cities. I would cut out at least one city. I haven't been to San Sebastian but I have been to the other places you named and they deserve more than 2 days each. Happy planning.
I would stick with Barcelona and Cantabria, if you want to take a pilgrimage to Santander. You can fly from Barcelona to Santander (for as low as €84), pick up a car and explore the region (quite beautiful).
Madrid, San Sebastian-Donostia and Sevilla deserve more time then you have this trip.
You can do it but you will only see the inside of a car or airports.
