Our three favorites were Bellavista (great views of Prague from outdoor patio), Orange Moon (Thai) and Chez Marcel (French).
We also liked the Noble Club, a wine cellar in Mala Strana for a little vino and cheese.

we were in prague 3 years ago. we loved our little hotel U Tri Bubnu . it is small and comfortable. very friendly. nothing fancy, but the most perfect location-- right on the central square. (at the back end.) you can walk anywhere easily from there.
by the way--if you are by any chance an opera fan,(or even if you're not...) the prague national theater is a small, opulent gem. we got tickets to the opera online... and they had them sent over to the hotel for us for no service fee!!

We stayed at the Hotel Kampa Garden on Kampa Island. It is located just a block off the far end of the Charles Bridge on quiet Kampa Island. The room was large, clean with a huge bathroom - very nice for our 5 night stay. We enjoyed the daily walk across the lovely Charles Bridge and the close convenient location. It was about $160. US$ in 2006. Good hearty breakfast and free use of a computer (good for e-mails).
check it out on Trip Advisor. You should be able to google it also.
I also recommend getting tickets for the National Theater (a beautiful, opulent interior) and the State Opera. We purchased for both the first day in Prague. ENJOY! The hotel also provides a contact for transfer from the train station. We got our own taxi when we arrived, but used the hotel driver when we left, and it was cheaper and nicer. Have fun.

I stayed 4 nights at the Hotel Residence Mala Strana in July, 2008.
I was satisfied.
The room had a small refrigerator, stove, and eating/cooking facilities.
Nothing great, but adequate for light meal preparation.
The breakfast was included in the room rate and it was very good. No quibbles at all there.
The rooms come in two sizes. I think for a stay of 4 or more nights that the larger room would be worth the extra expense unless you enjoy close association with your roommate.
The bed was comfortable, but the couch was really not very useful except to throw stuff on.
The bathroom was fine.
Even so it was a heck of a lot larger room that say the Kensington Close in London. Our room there could be more described as a slit, not a room. When I can stand between the beds and come within 2 inches of touching both walls with my arms outstretched, that is a narrow room!! The bathroom was an even smaller slit.
The potty was so tightly packed in that I am not sure a 300 pounders bottom could have made it all the way down! Perhaps greasing the walls would have helped.

Non-splurge meal choice: U Schnellu in Mala Strana. It is diagonally across from the tram stop in the square, on the "castle side". Reasoanbly priced. Get the Kozel dark beer.

I highly recommend the Arcadia Residence. Our family of three stayed at one of their apartments, the Kozna, from June 22-25th, 2008. The cost is 109 Euros per night including breakfast. The apartment was huge and could accommodate up to 5 people. It is convenient and centrally located in Old Town Square. In addition, the service provided by the owner (Mr. Pasquale Gusmano) was outstanding.
