Planning a trip to Germany, Poland, C-Republic, Slovenia and Croatia

We have 3.5 weeks total leaving USA on May 3 2012 How would you divide up our time? Here is what I was thinking
Fly to Berlin
Berlin 3 days total
Munich 4 days total (use these days to take day trips through rhine region) Or would you recommend moving from town to town to see this region)
Prague 3 days
Ljubljana-2 days
Lake Bled Region 2 days
Dubrovnik 3 days
Split 2 days
krakow 3 days
Warsaw 2 days
Our main focus on this trip is croatia and slovenia so I need help with those two places mostly.
that is what we are starting with but are very open to suggestions. Would you add time anywhere? Would you add places? What would you do to this iten? There are 4 of us we like scenic stuff, history and culture
