Need suggestion for 14 days trip to Italy includes Lake Como and ....

Hello Everyone, I am planning a anniversary trip for my husband and I for about two weeks in Italy.
We are around 55 years old and are very active avid travellers and have been in all over Italy as well except the areas on the Topic Title. Would love to visit Tuscany for few days ( have been in Tuscany before only for a day while our cruise was in the near area) but love to stay there for slow relaxing and exploring and then travel to Lake Maggiore, Cinque Terre and Portofino. It can be in any order and we can arrive in any major city that's closest to any of these location to start. Last year we did the the entire Amalfi Coast and loved every moment visiting all the cities.
Please advise what point to start and end and how many days in each area. We love to see villages and walk around , explore , see local tradition and mingle with local people. Also apreciate any hotel recomendation in each area, we are looking for a good location, cozy hotels mid to upper range. Look forward to all your wonderful suggestions and appreciate it very much.
