Need Itinerary Critique: From Ljubljana - Dubrovnik

In October for 2 weeks :
- Ljubljana to Lake Bled & Lake Bohinj
- to Plitvice
- to Hvar,Trogir,Split
- to Dubrovnik
We're active travellers in our early/mid 60s who love to walk & explore.How many days are needed for each area & please suggest most practical areas to base our touring from ? What is the best means of transport from area to area ? If by bus, how do we get schedules?
