Need advice on Travel passes in Paris and zones

I recently spent a month in Paris and I had a wonderful time. Despite mobility issues I exceeded my expectations.

I used buses to get around Paris as stairs in Metro were an issue for me.
At the risk of irritating Fordorities with yet another travel pass question ...
Because I am attempting to clarify this for friends who intend to travel to Paris I need to know if I am correct in the following conclusions and also answers to my remaining questions.

So Ticket t+
allows you to travel anywhere on the Metro regardless of zone
allows you to travel anywhere in zone 1 on the RER. Need destination specific ticket (point to point) for anywhere outside of Zone 1.
allows you to travel to/in which zones on buses?
Mobilis, Visite, ND
Allow you to travel anywhere within the zones specified
on Metro?
On Bus?
On RER?
Except Mobilis to/from CDG or ORY?
So if you have an ND Zones 1-2 and you wish to travel to Zone 3 to a specific location on RER or bus , you can modify your ND through machine or at cashier prior to boarding and again prior to boarding for return trip? I did this to go to St Germaine en Laye and back.
But how do you get back from Malmaison to La Defense on bus #258 legally on ND Zones 1-2? How could you modify it for the return trip? You can’t use a Ticket t+ to get to La Defense can you on #258 bus?

I have a feeling I returned to La Defense illegally!
