It's pretty straightforward if you use a site like Michelin or Mappy and plug in your car rental pick up point and your destination in Portofino.
If you are planning to drive to Portofino between now and the end of September, you could experience extremely heavy traffic once you reach the actual coast, in particular on the roads closest to Portofino, espcially if you are traveling on a weekend or from Aug 15 to Sept 1.
Know too that on the weekend of Sept 8, the coastal town of Recco -- which is one of the autostrade exits for Portofino -- hosts its annual town celebration, which paralyzes all traffic in and out of the town. If you are traveling on the weekend of Sept 8, bypass Recco for the Rapallo exit on the Autostrade, and drive through Rapallo to Portofino.
All that said, if there is any way you can avoid driving a car into the area of Portofino in summer you should. Instead, take the train from Milan to Santa Margherita Ligure and make your way from there to Portofino by boat, bus or taxi. If you need a car to leave Portofino, rent it in Rapallo, just 15 minutes away.
