Milan Entry Tickets and Transport Cards

I am reseaching entry tickets for attractions in Milan for our trip there next August. The attractions that interest us include the Duomo roof terrace, Leonardo's Last Supper, the Pinacoteca di Brera, the Pinacoteca Ambrosiana and Basilica d'Ambrogio. Maybe also Castello Sforesca and the churches St Maurizio and Santa Maria dell Grazia. It seems from the Milan Museum website that Last Supper tickets cannot be bought on their own. They have to be part of a package with something else, like a guided city walk, or one of the other attractions listed above. I was going through all the ticket combinations and prices, when it occurred to me there are undoubtedly fodorites out there who've done it already and who might also be able to give me tips that I won't find on the websites. So, can anyone help me find the best ticket combo to see as much as we can of the above at a not-too-crippling price?

If possible, advice on tickets, cards etc for Milan's transport system would be good as well. Does Milan have something equivalent to London's Oyster Card, for example? I found something called a Milano Card, but it seems to offer discounts on fares rather than function as an actual "transport card" in the way that the Oyster Card does.
