itinerary help; northern Spain & Portugal

I need help with an itinerary for northern Spain & Portugal. Mid-September; 10-12 days; Couple in early 60's. We like the country, hiking, mountains, agritourism; history, music. Madrid is our arrival and departure point but we don't plan much time there. We need help planning the trip outside of Madrid.
We might take public transport out of Madrid but eventually plan to rent a car. Would it be difficult to drive from Madrid ?
We see so many great things in photos : at least part of the Camino de Santiago, Picos de Europa, Coimbra,Portuguese national parks, the coast. We would like to get a taste of mountains, countryside and agritourism and some history. We prefer to spend 2 or 3 nights in the same place and not be constantly moving.
We are not afraid to go where English is not spoken.
Any suggestions for an itinerary that would give us a variety of the 2 countries?
