It depends on where you're from. There's a fast path for locals while outside of UK/EU have to go past the immigration official in London. Everyone must show passport to get on the train. I've never had my passport asked for on that particularly Eurostar route.

Wait a second, Grassshopper. I thought both France and UK immigration is done at Gare du Nord for London-bound trains so that you just walk the train at Waterloo...
I have never taken it, but that is my understanding.

If you do not have a visa for France you cannot take a train from Zurich to London, since you would have to travel through France. If your fiance just had an operation, it would not be a good idea to do extensive traveling by plane or by train...

If your concern is about your lack of a French visa, you will go through French passport control on the train from Zurich to Paris AND AGAIN getting on the Eurostar at Paris.
You will go through UK passport control in Paris, and you MIGHT go through it again at Waterloo. It's very common for non-Europeans to be subject to considerable interrogation at Waterloo.
If you haven't got a Schengen visa, you can't travel by land from Switzerland to Britain. Crossing France isn't the only route: but South Africans require visas for all the alternative land routes.
