Hotel recommendations for family of four (kids: 15, 12)

I've heard the hotel room occupancy rules/limits in Spain are more stringent than in the U.S.

Is it more cost-effective to book a quad room or two individual rooms? How readily available are the quad rooms (do most hotels have them)?

Are the occupancy policies different in the country vs. the major cities?

Do you have any specific recommendations for the following cities:

1. Barcelona
2. Hondarribia
3. San Sebastian
4. Madrid
