Have your hotel reserve a taxi for 3:30am unless there is a taxi stand in front of the hotel.
Most USA based airlines require you to check in at least 2-1/2 hours before your flight, but I not sure if there will be anyone at the airport in the dead of night.
Are you making a connecting flight through Madrid? If so, then you will only need to be at the airport about 90 minutes ahead of your scheduled departure. You'll need the time to check in (luggage), pick up your boarding pass and pass through security.

It might be well worth your while to attempt to find out at what time the check-in counter for your particular departure airline OPENS at BCN.
I would also recommend a taxi but I would also advise you to ask at your hotel how long they think it will take a taxi to reach the airport at that hour.
I would suspect no more than 30 minutes BUT we have no idea WHERE your hotel is located and that would have been helpful to know.

Dukey is correct about asking when the desk opens. Ours did not open until 5 AM, so it was a waste to get there earlier.
Not to make you paranoid, but our taxi driver booked by the hotel tried to cheat us by taking a circuitous, if scenic, route. I speak Spanish, so I called him on it, but believe me, you are stuck with the taxi you have at 3:30 because there aren't any others waiting for your business.

Taxi is the way to go. I too had to work this out last year in Barcelona. We timed it to arrive at 5:00 because my travel partner wouldn't leave any earlier. I was nervous. However! the counter didn't open until 5:00 anyway. The line was long and this made my partner nervous. I was ok however because everyone in that line was on the same plane. I don't know if you've been to this airport but it's small and easy to navigate.
