Monterosso has direct trains to Genoa and Milan - more to Genoa then Milan and it is a simple transfer. The other villages have shuttle trains to Monterosso, a single platform station where changes of train are easy.
There are direct trains Monterosso every two hours to Milano Centrale - according to www.bahn.de - the German Railways web site that I always use for any European train schedule as it is the easiest to use. A good way to access the Wunderbar www.bahn.de site is to go to the homepage of www.budgeteuropetravel.com and click on the link 'Best On-Line European Railway Timetable' (or some such wording) and this leads you to the English schedule page where you type in Monterosso (or other villages) and Milan and you get all the trains for the date you enter - I mention this homepage link because it also gives you several valuable tips on using www.bahn.de that may not be apparent at first encounter.
Those trains are mainly regional or IC trains that you can buy in any Cinque Terre station - no real savings for booking early - never a problem IME getting on. For good info on Italian trains check out www.seat61.com.
www.bahn.de shows one direct train from each village during the whole day so unless that one or perhaps two direct trains suit you just do the simple change of trains in Monterosso.
Or i can stay in La Spezia or Levanto and does those places have direct train to MIlan?>
Well just about any train that goes direct from La Spezia Centrale or Levanto to Milan will also probably stop in Monterosso - most folks would greatly prefer to stay in one of the fabled 5 Lands villages than La Spezia or Levanto, though those towns are not bad but just not the character of small villages on a rugged sea coast with no cars around, etc.
