Colosseum Underground Tour - slightly confused so looking for help!

Reading through the forums and visiting the http://www.coopculture.it, I understand that you need to ring them directly to book the underground tour...but before I do, I was wondering if anyone could help me out with some prior information?

How far can I book the tour in advance? We are travelling mid August this year.
How much is the tour? I know that we don't need to pay for our 5 children who are under 18's for the Colosseum but I am assuming we will for the tour?
What is the earliest time that they start (in English)?
How long do they last for?
Is the the best way/only way of touring the Colosseum?

I am sorry for all the questions but we are trying to be as prepared as possible - the only way to travel with less stress with such a large family! Thank you.
