You don't mention your interests or time periods but I can't imagine time better spent than to take a walking tour. We recently spent six nights in the city and found the intro tour with Insite Tours a great survey of the city and its history. Just show up at its meeting points and the well informed guides keep you interested for 3 1/2 hrs.
The museum scene is also fabulous with those on the Museum Island as well as many others--the Wall, the German History Museum, the Topography of Terror, the Jewish Museum and others.

Have been on 5 tours with Insider Tours and can highly recommend them.

Do go out to visit Sachsenhausen or Ravensbruck Concentration Camps. A visit to Track 17 would also be good, or to the Wannsee Villa. Take a 1-2 day trip to Potsdam.

Here are the places I enjoyed in Berlin and Prague:

Berlin:
I agree with the Insider Walking Tours, especially the intro tour. A tip about this tour which I did not understand until I took the tour - there are 2 meeting points (West and East). I thought they were 2 separate tours but they are combined. If you meet in the West (as I did), you hang around for the first hour. You meet your guide and chat for about 20 minutes, then go to the East meeting point where you meet up with the rest of the group and then hang around there for a while waiting for stragglers to join the group. If you don't want to waste time then meet in the East and show up 15 minutes late. The tour was excellent but the waiting around was not a good way to start the tour.

- Reichstag - one of the best things I did in Berlin; must book ahead
- Story of Berlin - the museum is nothing much but the Cold War bunker part (escorted tour only) was very interesting.
- Memorial to Murdered Jews - pictures do not clearly depict the significance of the memorial; you need to walk into it to understand it. Also spend some time in the welcome center which I thought was interesting.
- Checkpoint Charlie & museum
- Pergamon Museum for the Ishtar Gate and the Islamic art
- Boat ride on the river - there are lots of companies offering this ride and they leave at various times. Just walk along the river and find a boat that leaves when you're ready to take the ride
- Gemaldegalerie - fabulous Italian Renaissance art
- Walk around the Nikolaivertel area - one of the pretty parts of Berlin
- DDR Museum - interactive museum about life in the East
- Topography of Terrors
- Palace of Tears - I was the only person in this museum when I went. You only need about a half hour since it's small but quite interesting

One thing I would avoid is the Berliner Dom as it costs E7 to enter and was not a church worth paying the money.

Prague:

- Municipal House
- Mucha Museum
- Synagogues & cemetery
- Castle area
- Loreto Church
