I have read both booking.com and trip advisor. Both get good reviews so I'm torn. I have 3 children <7 and will be renting a van (Yikes!). I know the streets are small. Both hotels seem very accomodating in the size of their suites. Barock is significantly more expensive, but this is not a factor for me. I like that Alstadt is on the river, but no parking. Barock has agreed to let me park in front of their hotel, but we have no issues parking further if Alstadt is worth it.
I just want to know other opinions. Anyone else been to either hotel??

Altstadt Molitor DOES have parking. At least they did when we were there in September. I think the problem is that you will be in a VAN which is too tall for their parking garage.
I doubt you will find others on Fodor's that have stayed at the Molitor. When I was doing my research, I found nothing here.
It IS rated the #1 B&B on TripAdvisor but I really took a leap of faith and was rewarded.

We stayed at the Molitor a year ago December. It was a nice hotel but nothing out of the ordinary to our taste. It is handy to the old town and right across the street from our taxi driver's favorite Gasthaus, where we got a decent meal:

The Molitor had a pretty typical breakfast; the breakfast room was pretty small. The staff was very nice.
The building was recently converted to a hotel I understood and all the fixtures were fairly new.
The hotel was nicely located, though Barock am Dom is probably marginally better; they are only a few blocks apart. I would not say Molitor's closeness to the river matters much. The alleyways around the area are quite attractive. Our pictures:

I've stayed at both hotels actually and liked them both a lot. The hosts at both places are very nice and hospitable. The hotels are also very close to each other so there is really no difference in scenery. I think the quality of the rooms is the same. The breakfasts were comparable. I thought the Molitor was quieter. The Barock Am Dom is right beneath the cathedral so the church bells clanging in the morning is more prevalant at the Barock. Not an issue if you are an early riser but I can tell you after a night of Bamberg beer it made an impact on my head.
I have a feeling that based on your parking needs and the van that you may be better off at the Barock. Although there aren't many spaces in front of the building so I'm not 100% sure that is a guarantee.
If you stay at the Molitor you can park the van at the Geyerswoerth parking garage next to the tourist office and walk the fairly short distance over the bridge to the hotel. The cost isn't huge and since the hotel is cheaper then I suppose that is a wash.
It's a tough call. I guess if the difference in cost is a non-issue and you can get a parking spot for the van out front then the Barock Am Dom would be more convenient for you. The woman that runs the place is a spectacular host and I'm sure she'll do everything she can to accomodate you.
