14 Days Trip to Italy, Switzerland, France and or Amsterdam

Hi me and my husband are planning for a 14 Days Trip to Europe and We Want to Cover Italy, Switzerland and Paris and f Possible would love to spend a day or 2 at Amsterdam.
We leave Singapore on 29th June. In Italy we plan to Cover Vatican City, Pisa, Florence, Venice and Milan (like within 5 days) and on the 5th Day we plan to travel to Switzerland. We plan to Spend 3 days in Switzerland and then Plan to go to Paris and Spend another 3 Days there.

Can someone please suggest us a sample itinerary from your past experience. Also in Italy, do we need to book different hoels for different cities? Anyone providing some help on intercity transportation and inter-country transportation and where exactly should be stop for the day would be really helpful. We are on a moderate budget. Thanks.

P.S - In short we are grateful for whatever help / tips we get. Thanks in Advance.
