I'm planning a 4 night cruise to key west and cozumel out of Ft. Lauderdale Sept 21st. Do I need tirp insurance in the event of a hurricane? Also, have we been hit in recent years late September with a hurricane?

Trip insurance is always a good idea in case something happens to you or a loved one and you must cancel. If the hurricane is that bad, the cruise will be cancelled and you'll get a full refund.
Actually, a hurricane rarely results in a cruise cancellation but rather in a change of itinerary. And, yes, September is an active time for hurricanes.
Buy your coverage carefully as some policies do NOT cover "acts of God".
Cruise lines will very rarely refund your money. I cancelled a cruise two years September due to a hurricane. The cruise line did not cancel, did not offer a refund and the insurance did not cover our loss.
As a TA I can tell you NOT to expect the cruise line to do anything but sail in a different direction. That is not what happens.
